The global antibiotic resistance epidemic: how can we stem the tide?

International travel has increased over the last 20 years to the point where, in 2010 alone, international tourist arrivals were estimated to be 940 million globally.

This phenomenon, along with an increase in migrant workers, has led to unprecedented movement of antibiotic resistant organisms across international borders. One of the best examples over the last decade has been the worldwide dissemination of a particular resistance gene from the Indian Subcontinent, known as ‘CTX-M-15’.

CTX-M-15 is an “extended spectrum beta-lactamase” (ESBL) –  a class of gene that confers resistance to nearly all penicillin-like antibiotics and their derivatives, leaving very few antibiotic treatment options available.

Although CTX-M-15 is found mainly in Escherichia coli (E.coli), it is carried on mobile genetic elements known as plasmids, which readily transfer between different bacterial species. This makes the spread of CTX-M-15 particularly difficult to contain.

Many travelers returning from high prevalence regions such as the Indian Subcontinent, will carry CTX-M-15 in their gastrointestinal tract without symptoms for a period of time after their return. This allows an opportunity for further spread within their country of origin.

Very few travelers who are colonized on return are hospitalized while abroad, indicating that within high prevalence regions such as the Indian Subcontinent, CTX-M-15 is ubiquitous in the environment.

But recently, an even more troubling example has emerged – known as New Delhi metallo beta-lactamase (NDM-1). NDM-1 was first described as recently as 2009 in a traveler returning to Sweden from India. Since then there have been numerous reports documenting the importation of NDM-1 into various countries in every continent except South America, usually after travel to the Indian Subcontinent where NDM-1 is already widespread in the environment.
